First Visit

What Will Happen On My First Visit?

Please assist us at the time of your initial visit to the office by providing the following information:

  • Your referral slip and x-rays from your referring dentist, if available.
  • Completion of a health history form, including a list of all medications you are currently taking (please note that you can fill out your patient registration and medical history form on our website.)
  • If you have dental insurance, please bring any forms, insurance cards, or other documentation with you to the appointment.

Hours:

Monday-Thursday 8 a.m. to 4 p.m.; Fridays (alternating weeks) 8 a. m. to 2 p.m.

Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.

Please notify the office if you have a medical conditions, concerns, or prior surgery (for example artificial heart valves, joint replacement, or any conditions requiring pre-medication).

We will complete an in-depth medical and dental health history and a thorough examination to evaluate your periodontal health, measure attachment and bone loss, determine tooth looseness, evaluate your bite and TMJ health, and complete oral cancer screening.  Computer imaging will be used as a tool to help you better understand and visualize the findings, problems, and possible treatment options.  We feel very strongly that it is critical for our patients to have a good understanding of the disease process in order to achieve a successful outcome.
